A physician patient is a \textbf{participant-observer} of a discrete incident. It could be labeled as a \textbf{risk factor}} factor}  leading to a wound infection. There is a \textbf{thick description} of multiple contributory issues: apparent lack of a policy of enforcement of manners, resistance to information about infection, perhaps a lack of sensitivity to how such information may be heard by racial minorities, inability for the physician to obtain nursing cooperation, a common event. A historical perspective is added with comment on changes in systemic organization. The feelings in this situation could be better described. Clearly some confrontation is needed but it is awkward for all parties involved. It also is one of those "see something, say something" situations where saying something is politically incorrect and denied and dismissed.
