\section{Complex Context Critical Incident Reports}  These are examples of the most extensive and complicated type of report. The motivation to do this much work often comes from memories of major errors, serious impasse that frustrates the participants, exceptionally good examples of functioning organizations that possible could be replicated if we know what factors led to their success, and in general very memorable incidents good or bad. For naive users it is useful to use the note and narrative tools first a few times before trying to put it together in a CCCIR. Even then one should not expect it to lead to solving or closing the issue but it may provide enough further discovery, and good questions to keep one going in a difficult situation or to use to help others cope with poorly understood events.  \subsection{“The Patient Who Broke the System”}
