\subsubsection{\textbf{Organizational perspective:}}  A dependent and manipulative chronically suicidal patient has produced paralyzing inefficiencies, staff disruption and disorganization and high malpractice and litigation risk in multiple loosely connected public and social agencies. Inter-agency communication is impaired by lack of established channels, problems of medical confidentiality, threats by the patient of legal action and discouragement and helplessness of besieged caretakers.   \subsubsection{}  \textbf{Interpersonal perspective:} This patient’s manipulations are highly effective in engaging various “helpers.” She alternatively presents special appeals and threats engaging people in rescuing her and making demands upon others to take care of her, take her to and from hospitals, change her living situation or prevent her from committing suicide. Her dramatic history of multiple sexual assaults and “serious accidents” induces sympathy in the unwary and revulsion in the more sophisticated. Her success in manipulation at the individual level leads to splitting among personnel and agencies that lack both clinical understanding of her “modus operandi” and effective communication to present a united treatment plan.  \subsubsection{\textbf{Individual perspective:}}
